Baldur's Gate II. I don't know the exact count, but I have worn holes through my CDs of it from the sheer amount of time I played it.

All I really can relate to now (remember more like lol) is that I've played DW3/XL around 10 times altogether, getting all the special weapons and items before starting over. Though I'm talking over the years, I tend to start over and do everything from scratch so I have something to accomplish. I get bored with playing with maxed files after all.

Of all the Warriors games, 3 is my favorite and most played, it seriously makes me feel old when I think about the amount of peons I've killed.

Both of those about 3 times each
But the MOST are:
Final Fantasy IX (about 12-ish times)
Final Fantasy X (18-ish times)

Gotta love those great RPGs, you know?

Resident Evil 5 - at least 3 times

Played on normal the first time. played through the game again and tried to beat it less than 5 hours to unlock infinite rocket launcher. Third play through on Pro difficulty.

inFamous - 3 times
Played through both Good and Evil story so that's 2 times. 3rd play through was for trophy hunting (collected all the blast shards and the rest of the trophies). Honestly I wouldn't bother if the game wasn't fun although trophies did give me the incentive to play the game multiple times.

Uncharted 2 Among Thieves - 2 times
Beat the game on Hard. It unlocked Crushing difficulty. Didn't get quite enough so I played through Crushing again. Best SP experience I had on this gen so far.

Shing Force 2 nine times (fave game ever)
Final Fantasy VII five times
Kessen II four times
Shing Force three times

I dont actually replay the Warrior games much from fresh saves but i do play each character on each instalment through to the end... the only game i ever deleted my save and started from fresh was WO and that was because i was eager in anticipation for WO2 and wanted to refresh my memory on it all. I have however played but not completely to the finish many times the Dynasty Tactics games - they are underrated gems.

Final Fantasy X - ???
I'm not even going to attempt to guess the amount of times. I just keep trying different challenges every time I go through the game. I had to buy a second game disc because my first one committed suicide. Pansy.

Legend of Zelda: Majora's Mask - Too Much
FAVORITE GAME EVER. I love this N64 cartridge with my heart and soul. I've easily played through this game, I'd say in the range of 30's. I don't know why, I just love everything about this game, and it never gets old, at all. For anyone who's played this game before, you know it's bad when you can memorize the process of getting the Couple's Mask.

Disgaea 2 - 4 times
I'm not sure if this counts, I played through the story mode 4 times, but I'm still working on unlocking everything, which as anyone who has this can tell you, takes FOREVER. All I got left is beating Baal Castle on the second visit, getting a copy of each final weapon, and finding the Baal Pirates twice in the Item World in the Land of Carnage..... which makes me cry at the thought of it.

I don't remember a lot of games I played when I was young but in the past 2,3 years, the list stands like:

Growlanser V- Heritage of War: 4 times without the M2 amulet because otherwise there is no point in playing the game.

Chaos Wars: 3 times because I want to complete the photo gallery of each and every character.

Okami: 3 times because I just love playing the game...

Tomb Raider Legend: 10 times. When I have nothing to do or I feel bored, I play that. Takes around 4 hours to beat it now if I remember correctly.

Final Fantasy XII: 4 times because I can never bear it if my brother has better stats (of beating hunts and record time etc.) than me.

Kingdom Hearts 2: 3 times (twice 100%) because I love playing that game.

- Warriors Orochi (PS2) - Everything was 100% complete before I sold it. I must have played through each story mode a few dozen times just to max everyone to 99 ... and now I have the PSP version

- Samurai Warriors 2 (PS2) - Includes XL - 100% on everything here too.

- DW Strikeforce (PSP) - Completed story mode now 24 times to max 24 characters, cleared all DLC, Legend & Side missions multiple times and planning to continue this until all 40 characters are maxed

- Star Wars The Force Unleashed (Wii & PSP)

I'm sure there's more from my past, but I can't think of them right now

Romance of The Three Kingdoms XI - Plenty times. I can't even remember a specific number. If I had to guess I'd say 50 and over. Most of my walkthroughs are with the same force, being Cao Cao. I've only had about ten or so with anyone else. This one definitely has abundance of replayability if you're into the series.

Fire Emblem: Path of Radiance - 17. Long have the days passed when I would sit around for hours boss abusing... Good times with this game. Even if I would get screwed over by Rng it wouldn't matter. Somehow I'd get through it with my high leveled up characters. (Probably due to massive boss abusing in every file.)

Way of the Samurai - 33. Shortest game ever. The short story and plot mix this has to offer, has made games such as The Bouncer look as if you were playing Ninja Gaiden. But no, seriously at it's time this was one of my favorite games. Despite being a short game, the multiple endings and swords to master had you coming back for more.

Anything else not listed I either don't remember anymore, or has five or less walkthroughs. These three came out of the top of my head, well, because I recall playing them way too often. Tenchu: Wrath of Heaven might have gotten an honorable mention if I could only remember the precise number. It's been a while since I last touched that one.
